Document translation request

Providing translated documents for members with limited English proficiency (LEP) is necessary for effective communication and accuracy when obtaining healthcare information.

ODS Community Dental offers complementary document translation services for participating providers.

Documents for translation include:

• Medical history
• Consent forms
• Office policies
• Patient information forms and questionnaires

To request documents translation please contact odslangaccess@odscommunitydental.com

ODS is asking providers to update their websites to ensure members are offered webpage translation in their preferred language. A service such as Google Translate is suggested.

As a reminder providers must use the OHA approved financial waiver. Click here to access this document in multiple languages.
